Duplicate Referrals on the Referral page

Please when the member access to his Referral Page, he notices the same person repeated several times in the table.

How to correct this to view each member in one line of the table

Are you utilizing multiple plans or reentry features? If so, please try using the filter (in the dropdown) to determine if it has taken effect.
